Students - Please fill out this form if you require a schedule change.
NO SCHEDULE CHANGES ARE MADE ON THE FIRST DAY OF SCHOOL.

Please use the information on this page to complete the course request form. 

Course request delivery options: 
  • Upload documents one-at-a-time in our online enrollment system
  • Drop it off at the high school you are enrolling
  • Drop it off at your middle school (Kyrene School District and Tempe Elementary students only)
